Drug relationship is a subject about which most homeo practitioners are very much worried and confused. Some practitioners very much rely upon ‘drug relationships’ even in deciding their treatment protocols. Concepts such as ‘complementary’, ‘inimical’, ‘antidotal’ etc. are frequently utilized in everyday practice. Some doctors even deviate from the theory of similimum, due to their over indulgence with ‘drug relationship’ protocols. When prescribing a drug based on its so-called complementary relationship to the earlier prescriptions, we forget to consider whether it is a similimum by totality of symptoms. Yet, we call it ‘classical’ homeopathy. When searching through the literature and authorities regarding drug relationships, it will be seen that no serious scientific studies have been done on this subject. Most of the drug relationships are proposed by empirical clinical observations of practitioners, and not corroborated by scientific studies or evidences. More over, practitioners who are not much bothered over this relationships between drugs swear that their experiences prove otherwise. Some homeopaths prescribe so-called inimical drugs even simultaneously or alternatingly, and get expected clinical results.

For the last few weeks I was once again into an in-depth re-learning of ‘Chronic Diseases’. While carefully going through the initial paragraphs of that great text (Para 1 to 7), I was trying to follow the exact thought process of Dr. Samuel Hahnemann through which he finally arrived at his theory of ‘chronic diseases and miasms’. 
 
Imagine the desperation and hopelessness Hahnemann experienced over the disappointing outcome of chronic diseases treated on the basis of his original theory of ‘similia similibus curentur’. Listen these words: “their beginning was promising, the continuation less favorable, and the outcome hopeless.” 
 
Hahnemann confesses: “homeopathy failed to bring a real cure in the above-mentioned diseases, and to gain an insight more nearly correct and, if possible, quite correct, into the true nature of the thousands of chronic diseases which still remain uncured, despite the incontestable truth of the Homoeopathic Law of Cure, this very serious task has occupied me since the years 1816 and 1817, night and day”. 
 
I was really wondering about the dedication of our master living totally “occupied” with the “very serious task” of gaining “an insight more nearly correct and, if possible, quite correct, into the true nature of the thousands of chronic diseases which still remain uncured”. That too, the whole “years of 1816 and 1817, night and day”. 
Have you ever thought about the mental state of our master when he observed that patients “treated with such medicines as homeopathically best covered and temporarily removed the then apparent moderate symptoms” failed to make a permanent cure? His disillusionment to notice that the treatment on the basis of therapeutic law of ‘similia similibus curentur’ only “produced a kind of healthy condition, especially with young, vigorous persons, such as would appear as real health to every observer who did not examine accurately; and this state often lasted for many years.”? 
 
Hahnemann also had to witness the bitter truth that “the re-appearance of one or more of the ailments which seemed to have been already overcome; and this new condition was often aggravated by some quite new concomitants, which if not more threatening than the former ones which had been removed homeopathically were often just as troublesome and now more obstinate.” 
 
Hahnemann says: “when such a relapse would take place the homeopathic physician would give the remedy most fitting among the medicines then known, as if directed against a new disease, and this would again be attended by a pretty good success, which for the time would again bring the patient into a better state.

It may be an autoimmune disease associated with antibodies (vitiligo antibodies) to melanocytes.  Studies suggest there is some genetic mechanism involved in the etiology of vitiligo and that it is polygenic in nature.                     Melanocyte (melanophore) is a cell within the epidermis of the skin that produces the dark brown or dark pigment melanin. In any discussion regarding the ‘primary-secondary’ actions of homeopathic drug substances, the phenomenon of ‘opium causing excessive sleep and constipation, later followed by profound sleeplessness” is always cited as an example. 
 
Opium contains two main groups of alkaloids. Phenanthrenes such as morphine, codeine, and thebaine are the main narcotic constituents. Isoquinolines such as papaverine and noscapine have no significant central nervous system effects 
 
To understand the biochemistry of ‘primary- secondary’ actions of opium, we should learn the biochemical processes involving μ-opioid receptors. 
 
Read from Wikipedia: “The μ-opioid receptors (MOR) are a class of opioid receptors with high affinity for enkephalins and beta-endorphin but low affinity for dynorphins. They are also referred to as μ opioid peptide (MOP) receptors. The prototypical μ receptor agonist is the opium alkaloid morphine; μ (mu) refers to morphin 
 
MOR can mediate acute changes in neuronal excitability via “disinhibition” of presynaptic release of GABA. Activation of the MOR leads to different effects on dendritic spines depending upon the agonist, and may be an example of functional selectivity at the μ receptor. Perhaps we should now call the speed of light a suggestion? 
 
This is, of course, fascinating. But what does it say about the basic assumptions of those who cite impossibility as the reason that homeopathy cannot work? Clearly, the nature of reality is not perfectly known—and equally clearly, the presumption that homeopathy doesn’t work because it can’t work is a deluded self-referent argument. 
 
That argument is also utterly counter to the true spirit of science. Its purpose is to learn more about the world and universe we inhabit, not to make the arbitrary decision that we know it all, not even the basic facts. We do not. We must trust our senses. When what we see or experience doesn’t match what we expect, then the proper response is not to find excuses for why our impression was wrong. The proper response is to investigate, to find the truth. 
 
The Arguments Against Homeopathy 
 
After stating that homeopathy doesn’t work because it can’t, the rest of the arguments fall into line. They simply state, ad nauseam, that it has never been proven to work, that research that does show it works never happened or was deeply flawed, that research has shown that it doesn’t work, that its effects can be explained as placebo, that it hasn’t been through the rigors of double blind placebo controlled studies, that it can’t work because there is no active ingredient, and finally, that it can’t work because homeopaths cannot explain how it works.

I.Thumb sucking 
 
Thumb sucking is the act of putting the thumb into the mouth for a prolonged duration. It is considered to be soothing and therapeutic. Thumb sucking is generally associated with babies and young children. Children suck on objects to soothe themselves; sucking is one of a baby’s natural reflexes and completely typical for babies and young children.Thumb sucking can start as early as 2-3 months of growth in the womb or within months of being born. Most thumb-suckers stop gradually by the age of five years. Rarely does it continue into adulthood. It is not uncommon for thumb-suckers to suck both thumbs or their fingers. Finger sucking is synonymous thumb sucking in effect and treatmentthumb-sucking can cause problems for dental development. The only time it might cause concern is if it goes on beyond 6 to 8 years of age. At this time, it may affect the shape of the oral cavity or dentition. The severity of the problem depends on frequency, intensity, duration and also the position in which the thumb is placed in the mouth. The relationship between the upper and lower jaws may also be affected. Speech defects can occur from malaligned teeth resulting from thumbsucking finger-sucking.

II.Stammering 
 
Stammering is the inability of a person to speak in a typically fluent manner. A person who has a stammering problem will experience hesitancies, silent pauses and significantly frequent struggled behaviours in the form of repetitions, prolongations and hard vocal attacks. These are often accompanied by facial grimaces, hand jerks, tongue thrusts, etc. They frequently avoid speaking situations such as introducing themselves, answering phones, talking in a classroom situation and making presentations. Stammering in a young child may be determined by genetic factors. Stammering may become progressively worse in response to family stresses, listener reactions, and even the ….of learning speech and language. Performance pressure, dramatic changes in the environment, Some of the common precipitating factors are: change of schools, moving homes, birth of a sibling, separation of parents, conflict in the family, use of inappropriate speech and language. If your child speaks like this: "Mmmmmmummy, come here", then you need to seek professional assistance immediately. 
 
Homoeopathic Treatment: Good IPR with child & constitutional treatment Like Strammonium, Natrum Mur, Thuja, Baryta Carbonicum, Magnesium Carbonicum, Lycopodium. Argentum nitricum. 
 
III. Bed wetting: 
 
Bedwetting is common in children, and can affect adolescents too. It affects twice as many boys as girls, and can cause embarrassment and stress for children and their families. The ability to control the bladder may take longer to develop in some children. It is rare for bedwetting to be caused by a medical problem. There is often a family history of bedwetting. Most children are 'dry' during the day by the age of three and most are 'dry' at night by school age. It's considered normal for children to occasionally be wet at night until they're about five.

The softer bones seen in persons with osteomalacia have a normal amount of collagen, which gives the bones its structure, but lack the proper amount of calcium.There are numerous causes of osteomalacia. In children, the condition is called rickets and is usually caused by low levels of vitamin D. 
Other conditions that may lead to osteomalacia include: 
 
•Not enough vitamin D in the diet 
 
•Not enough exposure to sunlight, which produces vitamin D in the body 
 
•Malabsorption of vitamin D by the intestines

Definitions 
 
 
Clinical Definition 
Asthma is a clinical syndrome defined physiologically by episodic reversible airway narrowing (Psora) and hyperresponsiveness of the airways to a variety of stimuli (Tubercular). 
 
Pathological Definition 
It is defined pathologically by the presence of certain recognizable microscopic features including infiltration of the airways with eosinophils (Psora), hypertrophy and hyperplasia of airway smooth muscle (Sycosis), hypertrophy and hyperplasia of mucous secretory apparatus (Sycosis), and overall thickening of the airway wall (Sycosis). 
 
Prevalence 
 
It affects men and women equally. The prevalence of asthma below age 20 is greater than that above age 20. 
 
Mortality 
Mortality rates from asthma are difficult to estimate because of the difficulty of establishing asthma as the cause of death. 
 
Pathophysiology 
 
Alterations in Airway Constitutive Cells 
 
Both hyperplasia and hypertrophy of the airway epithelial cell layer (Sycosis) are present in asthma and contribute to the thickening of the airway wall (Sycosis). Within the epithelial layer there are increased numbers of surface secretory cells (Psora) as well as hypertrophy and hyperplasia of airway mucus glands (Sycosis). Thickening of the airway smooth muscle layer (Sycosis) also occurs. These changes result not only in thickening of the airway wall, which promotes airway hyperresponsiveness (Tubercular) on a simple mechanical basis, but also in an altered phenotype of the resident cells, which produces a microenvironment whereby activating stimuli enhance the production of prophlogistic mediators and cytokines (Psora). These mediators and cytokines in turn contribute to the airway obstruction and hyperresponsiveness (Tubercular) that characterize asthma. 
 
Infiltration by Inflammatory Cells 
 
The airway wall in asthma is infiltrated by T lymphocytes (Psora). 
Alterations of the Noncellular Component of the Airway Wall 
The airway wall is thickened (Sycosis) in asthma. The basement membrane is increased in thickness (Sycosis) and exhibits alterations in the structure of its collagen components (Psora); such thickening promotes airway obstruction and hyperresponsiveness (Tubercular). Liquid that infiltrates the airway wall and surrounding tissues as a result of local inflammation (Psora) further amplifies airway obstruction.

A lot of confusions and phobias exist regarding the selection of potencies to be administered, after selecting the similimum. Everything is controversial in this area of applied homeopathy. Each homeopath has his own ways, and believes that only he is right.Young homeopaths get confused due to totally contradicting advices they get from their different teachers. 
 
Nobody knows what exactly happens during potentization. Nobody knows what are exactly the active principles of potentized drugs. Nobody exactly knows the molecular mechanism by which potentized drugs interacts with biological organism and creates a therapeutic effect. Nobody knows how lo potencies are different from high potencies. Everything is based on speculations. Dynamism, vibrational theory, Vitalism- everything explained as phenomena happening 'beyond science'. 
 
I am trying to resolve the issue of potencies in the light of scientifically viable working hypothesis regarding homeopathic therapeutics and potentization. 
 
According to my concept of ‘molecular imprinting’ involved in homeopathic potentization, the active principles of potentized drugs are ‘molecular imprints’ of constituent drug molecules. Since a drug substance constitutes diverse types of independent molecules in it, potentized drugs also would contain different types of ‘molecular imprints’ representing those different drug molecules. These ‘molecular imprints’ acts in their independent capacity of configurational similarity by binding up on pathogenic molecules, producing a therapeutic effect. 
 
As per this view, molecules of drug substances would be completely removed from the medium when the dilution crosses Avogadro limit. That means, even the smallest sized drug molecules will disappear above 12c potency. Hence, I proposed that 12c will be the ideal potency for therapeutic purpose, and further higher potencies will not be different from 12c in medicinal property. Since drug molecules will be absent above 12c, I presumed that there is no meaning in continuing potentization higher and higher. On that basis, I suggested to use 12C to 30c potency only.

Since CCRH has seriously undertaken drug proving as part of their research projects recently, I would like to bring some relevant points to the notice of researchers and authorities there. 
Many drugs are proved in molecular forms (mother tinctures and potencies below Avogadro limit), whereas certain other drugs are proved in potencies above 12c. 
 
My doubt is, whether the symptoms produced by same drug in high potencies and molecular forms will be similar? If they are different, how can we decide which symptoms are to be given more importance in the selection of similimum? I request the CCRH authorities to resolve this problem by conducting drug proving of same drug in both ways, and doing a comparative study of symptoms. 
 
According to scientific view, ‘Similia Similibus Curentur’ means: ‘diseases caused by specific molecular inhibitions and expressed through specific groups of subjective and objective symptoms can be cured by potentized forms of drugs that could create similar pathologic molecular inhibitions and symptoms in healthy individuals if applied in crude form’. Same can be stated in a different way as: "pathological molecular inhibitions can be rectified using ‘molecular imprints’ of drug molecules that can create similar molecular inhibitions if applied in molecular form”. 
 
Homeopathy utilizes ‘drug proving’ for studying the pathogenic properties of drug substances by observing their capacity to produce various pathological symptoms in healthy organisms. Homeopathy is based on the principle that a substance becomes a medicinal agent only because it has some disease-producing properties. In other words, if we could know what pathological inhibitions and symptoms a drug can create in healthy organism, we can decide in what disease states that drug could be used as a therapeutic agent in potentized form. Homeopathic theoreticians till date try to explain the ‘modus operandi’ of potentized homeopathic medicines using one or other hypotheses available or evolved by them, and as such, homeopathy still belongs to a class of ‘unverified science’. 
 
By the term ‘hypothesis’ we mean a ‘proposed explanation’ or "educated guess" for a phenomenon that we observe around us. Every ‘proposed explanation’ cannot be considered a ‘scientific hypothesis’. To be a ‘scientific hypothesis’, the scientific method requires that one can test the hypothesis using available scientific tools and methodology. Every new scientific hypotheses are generally based on previous observations that could not be satisfactorily be explained with the existing scientific theories. The words "hypothesis" and "theory" are often used synonymously in common and informal usage, even though a ‘scientific hypothesis’ is not exactly the same as ‘a scientific theory’. A hypothesis should be proved 'using scientific tools' in order to become a scientific theory. A ‘working hypothesis’ is a provisionally accepted hypothesis that is ready to be proved. Experimenters will have to test and reject several hypotheses before solving the given problem ultimately. 
 
Testability (using existing scientific tools), Simplicity (avoiding excessive numbers of entities), Scope (apparent application of the hypothesis to multiple cases of phenomena), Fruitfulness (hypothesis may help to explain further phenomena in the future), and Conservatism (fitting with existing recognized knowledge-systems) are considered to be the essential qualities of a good scientific hypothesis. 
 
Viewing from this standpoint, it is very much clear that most of the presently existing most celebrated 'theories' or hypotheses regarding homeopathy cannot be considered ‘scientific hypotheses’ since they contain concepts and conclusions that ‘could not be tested by any scientist using currently available scientific tools and methodology’ or 'fit with existing recognized knowledge-systems'.

It is marked by transient attacks of acute arthritis initiated by crystallization of urates within and about joints leading eventually to chronic gouty arthritis and the deposition of masses of urates in joints and other sites creating tophi. Tophi represent large aggregates of urate crystals and the surrounding inflammatory reaction (see later). Most, but not all, patients with chronic gout also develop urate nephropathy. Gout with nodosities (tophi) <o:p></o:p></div><div class="MsoNormal">                  Gout with appearance of nodes, knots around the small joints of finger. Gouty arthritis deformans <o:p></o:p></div><div class="MsoNormal">                    A gouty disease in which there is inflammation around the joint, inflammation of a joint with deposition of salts in the affected joint. Such deposition causes the muscles to tighten and further can lead to deforming of the joint in one direction or bending of the joint to one side, thus it is called as gouty deformans or arthritis deformans.
