We hold that force, on energy, or Dynamis, whatever you wish to call it, is the law of nature; it may express its powers in different but in its essence it is that which was breathed into man at his creation which made him a living soul, name of author (yr of publication). 
 
Crude drugs have three degrees of action: 
1. Mechanical 
2. Chemical and 
3. Dynamic 
 
The first two grades are of little comparative note in homeopathy. This is demonstrated by the fact that their provings in crude form produce comparatively little of worth, where as in those provings made from the thirtieth and above we obtain more complete provings because of the dynamic action. The full power of the drug in its dynamic action is brought out in the potency; where as the grosser material elements by their crudity develop no fine individualities in their provings. Entirely new activities are developed, liberated and may be transmitted and changed into the poetizing medium. (Cf. organon 269). This is shown by the potentization of inert substances like Carbo veg, Lycopodium and Silicia 
 
There is very little direct evidence that the biological response is produced by a chemical reaction between the drug and the cell constituents. This assumption is chiefly justified by the facts that it is supported by much indirect evidence, and that there is no alternative hypothesis has stronger evidence in its support. If assume that there is some simple relation between the amount of drug entering into combination and extent of the biological response, then the following consideration must apply. 
 
If the fundamental chemical reaction consists in fixation of drug by some particular constituent of the cell, then this must result in a reduction both of the free drug and of the free receptors in the cell. The relative amounts of drug solution and cells can, however, be varied at will, and hence the concentration of drug can always be kept constant by proving a sufficient excess of drug solution; indeed in many cases of the drug fixed is so minute that it is so difficult to reduce the drug solution to a volume small enough to show changes of concentration (p. 60). 
 
“The simpler the conditions chosen and the smaller the difficulty in securing access to the inner surface, the quicker is the absorption ... in the case of well powdered absorbents the equilibrium is mostly reached in seconds or minutes…” 
 
The action is much more rapid in the case of well powdered substances (Triturations).

The first homeopathic software David Witko created was CARA which started as a software system for the Royal London Homeopathic Hospital who wanted to repertorise using Kent's Repertory. In those early days I worked closely with Dr Michael Jenkins and we worked hard to make CARA easy to use. In fact the name chosen, CARA, means 'friendly' in Gaelic. Throughout the 80's and 90's CARA continued to develop and grow. During this time CARA gained the reputation of being really easy to use and a cost-effective way of getting started with homeopathic computer software. Subsequent upgrades and developments saw it transform into a beautiful software tool, used by thousands of homeopaths all over the world. 
 
At the start of the 21st century and as Miccant approached 25 years of working for the homeopathic community worldwide I decided the time was right to create a new homeopathic software program. This work took 2 years of my life and has been named ISIS.

HENNAI: More than 1,100 students of Ayurveda, Siddha and Unani courses of Tamil Nadu Dr MGR Medical University are planning to boycott classes from Monday as they fear their courses will soon become invalid after portions pertaining to allopathic medicine were removed from their syllabus. The Central Council of Indian Medicine (CCIM), a regulatory body for the Indian medicine doctors has threatened to withdraw recognition to the state medical university, which removed allopathic content from their syllabus for traditional medicine. The council's one month deadline to withdraw its decision on syllabus revision ends September 17. 
 
Students from Government Siddha College in Palayamkottai, who went on a token fast last week, have threatened to boycott class indefinitely. They argue that the council had allowed students of Indian medicine to also study parts of modern medicine to enable them to become better doctors. The university has deleted surgery, pharmacology and large sections of orthopeadics and ophthalmology. "If the university is not recognised, we will not be registered as doctors," said Sabari Manikantan, a student at the Government Siddha College, Palayamkottai. Homeopathy is based on the principle that a substance becomes a medicinal agent only because it has some disease-producing properties. ‘Constitutions’, ‘constitutional symptoms’ and ‘consitutional drugs’ are concepts which play a very important role in homeopathic theory and practice. Concepts such as ‘genetic constitution’ and ‘miasmatic constitution’ are frequently heard in homeopathic discussions. There have been a lot of attempts to explain constitution in terms of 'miasms', genetics, embryology and many other concepts. 
 
I am trying to evolve a scientifically viable understanding of our concept of 'constitution'. I think it would be more logical and scientific if we understand 'constitution' in terms of ‘phenotypes’ of individuals. To understand and explain ‘constitutions’ in scientific terms, we have to understand the concepts of ‘genotypes’ and ‘phenotypes’ in modern genetics. 
 
According to modern genetics, the ‘genotype’ is the ‘genetic substance or ‘DNA’ inherited by the organism from its previous generation. It is called the ‘genetic blue print’. 
 
The ‘genotype’ contained the organism gives rise to individual ‘phenotypes through ‘gene expressions’. The ‘genetic code’ stored in DNA is interpreted by ‘gene expression’, and the properties of these expressions five rise to the ‘phenotype’ of the organism. 
 
A ‘phenotype’ is the observable characteristics or traits of an organism, such as morphology, development, biological and physiological properties, behavior, and products of behavior. ‘Phenotype’ is the result of ‘gene expressions’, which is decided by the interaction between genetic blue print and environmental factors.

‘Genotype’ of an organism is the inherited instructions it carries within its genetic code. Organisms with same ‘genotype’ do not appear or act the same way, because its ‘phenotype’ is decided by the interaction with environmental and developmental conditions. Similarly, not all organisms that look alike necessarily have the same genotype. 
 
This understanding of ‘genotype-phenotype distinction’, proposed by Wilhelm Johannsen in 1911 to make clear the difference between an organism's heredity and what that heredity produces, is very important in providing a scientific explanation for the homeopathic concept of ‘constitutions’. 
 
Despite its seemingly straightforward definition, the concept of the phenotype has some hidden subtleties. Some would argue that anything dependent on the genotype is a phenotype, including molecules such as RNA and proteins. Most of the molecules and structures coded by the genetic material are not visible in the appearance of an organism, yet they are observable and are thus part of the phenotype. Human blood groups are an example. Others would say that this goes beyond the original intentions of the concept with its focus on the (living) organism in itself, meaning that the lowest level of biological organization compatible with the phenotype concept is at the cellular level. Either way, the term phenotype includes traits or characteristics that can be made visible by some technical procedure. Another extension adds behavior to the phenotype since behaviors are also observable characteristics. Indeed there is research into the clinical relevance of behavioral phenotypes as they pertain to a range of syndromes. 
 
Phenotypic variation (due to underlying heritable genetic variation) is a fundamental prerequisite for evolution by natural selection. It is the living organism as a whole that contributes (or not) to the next generation, so natural selection affects the genetic structure of a population indirectly via the contribution of phenotypes. Without phenotypic variation, there would be no evolution by natural selection. 
 
The relationship between ‘genotype’ and ‘phenotype’ has often been conceptualized by the following relationship: “genotype (G) + environment (E) + genotype & environment interactions (GE) → phenotype (P)” 
 
‘Genotypes’ often have much flexibility in the modification and expression of phenotypes; in many organisms these phenotypes are very different under varying environmental conditions. The concept of phenotype can be extended to variations below the level of the gene that affect an organism's fitness. For example, silent mutations that do not change the corresponding amino acid sequence of a gene may change the frequency of guanine-cytosine base pairs (GC content). These base pairs have a higher thermal stability than adenine-thymine, a property that might convey, among organisms living in high-temperature environments, a selective advantage on variants enriched in GC content. 
 
A phenotype is the ensemble of observable characteristics displayed by an organism. The idea of the phenotype expresses all the effects a gene has on the outside world that may influence its chances of being replicated. These can be effects on the organism in which the gene resides, the environment, or other organisms. 
 
Gene expression is the process by which information from a gene is used in the synthesis of a functional gene product. These products are often proteins, but in non-protein coding genes such as ribosomal RNA (rRNA), transfer RNA (tRNA) or Small nuclear RNA (snRNA) genes, the product is a functional RNA. The process of gene expression is used by all known life to generate the macromolecular machinery for life. 
 
Several steps in the gene expression process may be modulated, including the transcription, RNA splicing, translation, and post-translational modification of a protein. Gene regulation gives the cell control over structure and function, and is the basis for cellular differentiation, morphogenesis and the versatility and adaptability of any organism. Gene regulation may also serve as a substrate for evolutionary change, since control of the timing, location, and amount of gene expression can have a profound effect on the functions (actions) of the gene in a cell or in a multicellular organism. 
 
Factors, such as such as miasmatic, environmental, nutritional, occupational, infectious, emotional, ontogenic, metabolic and xenobiotic influence the process of ‘gene regulation’ at various stages of ‘gene expression’, through which the particular ‘phenotype’ or ‘constitution’ of the individual organism is determined. As such, ‘constitution’ of an individual is the ‘phenotype’ determined by the ‘protein constitution’ developing through ‘genetic expression’’Constitution’ is expressed in the form of totality of general physical symptoms, morphology, mental symptoms and behavioral peculiarities.

New drug hope for cancer patients

KOLKATA: Diagnosed with stage-IV pancreatic cancer in 2001, Minu Dutta was in such an advanced stage of the disease that neither surgery nor chemotherapy was possible, and her expected survival was only about three months. As a last resort, she enrolled herself into a clinical trial of psorinum therapy. Complete tumour remission was observed within one year of treatment. After 10 years of diagnosis, she is still disease-free. "This is a new life for me," she gushes with relief. 
 
Psorinum therapy is an active-immunotherapeutic anti-cancer treatment against six types of cancer - liver, gall bladder, stomach, pancreatic, lung and oesophagus. The key medicine is Psorinum 6x - a diluted alcoholic extract of pus found in scabies vesicles. The medicine is liquid in form and administered orally. 
 
"According to the pre-clinical data, Psorinum 6x activates different immune-effector cells, for instance T-cells, and accessory cells like macrophages, dendritic cells and natural killer cells that can trigger a complex anti-tumor immune response," explains Dr Aradeep Chatterjee, who is also the research director of the Critical Cancer Management Research Centre, Kolkata. 
 
Kashinath Saha's case was very similar to Dutta's. In 2003, he learnt that he had stage-IV lung cancer and was told that his expected survival was only about five months. "I had given up on life. I couldn't afford the expensive chemo and everything. So I thought this is the end." He was one of the 95 lung cancer patients who participated in another of three clinical trials on Psorinum Therapy. "I am now cancer-free and leading a normal life," he added.

The cases of Dutta and Saha were reviewed and verified by the National Cancer Institute under the National Institutes of Health of USA as part of their Best Case Series Programme. 
 
Psorinum therapy, however, has faced some skepticism among a few oncologists in the city. Dr Goutam Mukherjee says: "It is doubtful how far Psorinum can work independently without conventional lines of treatment, like chemo or radiation therapies." 
 
However, in the three clinical trials conducted till date to evaluate the efficacy of psorinum therapy, the participants' eligibility criteria included pathological "confirmation of malignancy, inoperable tumour, and no prior chemotherapy or radiation therapy". The details of all the three trials were reviewed by the American Society of Clinical Oncology (ASCO) and the outcomes published in premier peer-reviewed cancer journals including the Journal of Clinical Oncology (2009, 2010 and 2011). The first clinical trial involved 158 participants with advanced liver, gall bladder, pancreatic and stomach cancers, the second trial was conducted on 95 participants with lung-cancer and the third trial was on 65 oesophageal cancer patients. 
 
Dr Jaydip Biswas, director, Chittaranjan National Cancer Institute, Kolkata, commented: "The findings have been exciting and the journals support the clinical efficacy of Psorinum 6x, and confirm that it has no adverse side-effects." A section of experts, including Dr Amitava Chakraborty, assistant professor, department of radiation oncology at RG Kar Medical College, Kolkata, hail this novel treatment protocol as "a revolutionary work in cancer research". 
 
University of Texas MD Anderson Cancer Center, Houston, USA, has signed a "mutual confidentiality agreement" on July 11, 2011 with Dr Aradeep Chatterjee to initiate follow up clinical trials on this novel cancer treatment protocol. Dr Siqing Fu, Assistant Professor at MD Anderson, recently corresponded, "We are very excited to collaborate with Dr Chatterjee to bring psorinum 6x into allopathic cancer therapy." The therapy is already being replicated in Denmark. 
 
Oncologist Dr Mikael Nordfors at Humlegaarden International Cancer Centre, Copenhagen, Denmark, appears very convinced about the efficacy of this therapy: "Since I started with Psorinum Therapy, I've experienced a big improvement in results. I consider it to be perhaps the single most interesting treatment method for cancer that exists today," he said. 
 
Each cycle of three months of the treatment costs only about Rs 3,500. A cancer patient needs approximately eight to 12 cycles of the treatment to complete a full course. According to Dr Aradeep Chatterjee, "Randomized, controlled clinical trials are now being planned to determine if psorinum therapy is better than the chemotherapies to treat those six types of cancer." If psorinum can live up to its promise, cancer patients around the world will have cause for relief, and cancer will lose its much dreaded edge, for sure.

Understanding rubrics is the first step towards a successful selection of remedy. One must be thoroughly familiar with the stock of rubrics so far as their meaning is concerned. Here a good repertory is of great help. No matter which repertory we use, it will never, ever give us the right remedy in a straight shot. So, we must use the repertory from the perspective of a certain philosophy and principle, and never use it simply as a mechanical tool. 
 
Selection of the right remedy depends on many factors: 
 
The patient’s narration or expression of their symptoms: Here we must know how to convert the patient’s symptoms into the language of repertory. 
 
The physician’s ability to make keen observations of symptoms. 
 
The physician’s skill in interpreting symptoms: The best interpretations come from understanding the situation of a patient. This usually must be understood in the form of a story or in a form of an evolution. Let’s take an example. 
 
A boy wants to become an artist. From a very young age he loves to read books related to the arts, he loves to draw and paint, and is very excited on seeing related television programs. He is enthusiastic about taking part in art competitions and he loves to visit museums and art galleries. Added to this, he has a very dominating father. The father says “Look, you cannot make money by studying at a school of arts. You have to study business so you can get a good job and make lots of money.” Then conflict starts in the mind of the child, regarding his burning desire to be an artist against the restrictions from his parents to pursue commercial education. 
 
Now, how are we going to interpret these symptoms? Never assume you have a correct interpretation of the symptom without confirmation from the patient - that is, never ever interpret from your own imagination.

In the case given above, we must understand that the child wants to do something but the dominating parents restrict him – and ultimately the child surrenders. 
 
In this case the rubric we select, may be 
 
‘Mind-contradictory, actions are contradictory to intentions’ 
 
or it may also be ‘Antagonism with self’ 
 
or we may also take the rubric ‘Ailments from domination’ 
 
Once we decide on one of these rubrics, we must confirm its accuracy with the patient. 
 
Then we take it in a case analysis. 
 
Sometimes there are cases where it is difficult to find the exact feelings of the patient in the form of a rubric. Let’s take the example of a person who is in many ways an 
 
extrovert, but he does not share his feelings with anybody. Is it correct to take the rubric Reserved? Or should we take Reserved displeasure? Because this person is primarily an extrovert, he is not well described by these choices. So the correct rubric may well be ‘Hides his inner feelings’ 
 
As another example, consider a person who has a strong aversion to quarrels, violence, arguments etc. Here, it is not always correct to take ‘Quarrel, aversion to’. Depending on the situation, ‘desire for harmony’ may be a better rubric. 
 
The next step is the use of the rubrics in metaphorical way, in expanded sense. For this we must look to the surroundings, keep our eyes and ears open during our day to day conversations with people. One should proceed from meaning to expressions. Merely collecting expressions and finding their equivalent rubrics is of help but to some extent only because the expression may amount to any one of several rubrics or more than one rubric. Likewise one rubric may have a variety of expressions. So, we must have a thorough knowledge of how to differentiate the rubrics which seem to have a somewhat similar meaning. 
 
Here’s an attempt: 
 
*Abusive, Cursing & Contemptuous’ 
 
* Abusive: One should consider this symptom in conjunction with the heading ‘Cursing’ in order to distinguish between the two. Both concern verbal attacks on people or things. The abusive person uses rude and insulting words in order to offend others, whereas cursing is the vivid expression of surprise or annoyance. In assessing these symptoms, one must bear in mind the patient’s education & background. Here we must know how to differentiate this rubric from ‘Contemptuous’. The contemptuous person not only belittles other people but also feels superior to them. In his scorn there is indifference, rejection & sometimes even loathing. The symptom is most easily recognizable in racists who despise any racial group to whom they feel superior. We can differentiate it with ‘Rudeness & Insolent’ as well. The rude person is impolite and coarse in word & deed. Like the insolent person he has absolutely no consideration for anyone else, but the rude person adds a dash of crudeness & bad taste whereas insolence is a lack of respect. It may be detected in patients during consultation, although few will admit to it. Look for an attitude of excessive casualness & confidence, the impertinence implicit in all the movements. This person is ill-mannered & completely lacking in modesty. 
 
* Fanaticism, Anarchist & Revolutionary 
 
A fanatic person may be readily identified by his attitude & behaviour. Within a group he will be passionately enthusiastic in support of an idea. The greater the validity of an objection, the more aggressive and dogged will be his defense of them. He would rather die than admit that he might be wrong and his adversary right. This may lead him to compound his problems to an absurd degree, rather than to try to solve them. His blindness and lack of flexibility prevent him from clarifying his on thoughts or from exchanging ideas. He shuts himself off from all other points of view. 
 
Fanaticism is more often used in context with religious mania. 
 
Anarchist: An Anarchist means one who rebels against any authority, established order or ruling power or one who uses violent means to overthrow the established order. Rejection of all forms of control & authority. 
 
In terms of intensity of obstinacy or craziness an Anarchist is more intense than a Fanatic. A fanatic can not be an anarchist but an anarchist can be a fanatic. 
 
If intensity increases to yet another higher degree then an anarchist becomes a revolutionary. 
 
Fanaticism: Aur-ar, Caust, Lach, Puls, Rob, Sel, Sulph, Thuja 
 
Anarchist: Arg Nit, Caust, Kali Carb & Merc 
 
Revolutionary: Merc 
 
If we have a look on the medicines mentioned, we can easily predict that how a Causticum Fanatic (grade1) converts into an Anarchist (grade 2) & a Merc Anarchist (grade 2) converts into a revolutionary (grade 3). 
 
We can also co-relate it miasmatically, 
 
Fanaticism ————————– Psora

* Frivolous, Foolishness & Childish behaviour 
 
Frivolous means: Unworthy of serious attentions. Trivial nature of anything he comes in contact with. 
 
The frivolous person is vacuous & superficial. He is devoted to casual relationships, drunkenness, debauchery, fine cigarettes, wild parties & pornographic films. His sole ideal is the pleasure of empty distraction. He achieves nothing important or substantial. 
 
A frivolous person might narrate his complaints with laughing as if he were not very serious about it. 
 
We must know how to differentiate this rubric with foolishness & childish behaviour. Foolish or clownish behaviour in children is easily observed during the consultation. These attempts to be funny or amusing often indicate that the child has low self-esteem, or it may be a means of calling for attention. 
 
A frivolous person knows the after-effects or ill-effects of his negligence but still he doesn’t cares about that, whereas a foolish person don’t even know what could be the ill-effects. 
 
The symptom also occurs in patients with an emotional immaturity (Pulsatilla, Ignatia), exaggerated fixation on parental images, need for protection, emotional instability, egoism, jealousy, difficulty in resolving problems etc. Childish behaviour should not be confused with ‘Imbecility’ or with ‘Idiocy’ which involve mental disability. ‘Imbecility’ covers a mental age of between three & seven years, and ‘Idiocy’ one of not more than three years. 
 
So, a frivolous person is one who does not appreciate the serious nature of anything he comes in conduct with – In appropriately silly. 
 
* Fastidious, Censorious & Conscientious *Fastidious: means difficult to please or suit. Delicate to a fault. Everything has to be “just so”. “Disdainful; squeamish; rejecting what is common or not very nice; suited with difficulty”. This person is excessively scrupulous and meticulous. He gives careful attention to details, (another PERFECTIONIST) and is excessively concerned with cleanliness. *Censorious* Censorious is a person who see every action of others with critic eye especially for the sake of disapproving ,censorious can be for any behaviour, personality, structure, animate or inanimate thing and he has no fixed standards to be met with . while on contrast Fastidious has some standards to be met with and is not pleased till the parameters set by him are fulfilled and these standards are usually for inanimate objects like fastidious for dressing, cleanliness, taste, time, order of things and he gets satisfied when his standards are met with while censorious is always criticizing others and most of times like capricious, he himself never knows what is right but he 
 
would say that i dont know what is right but this is wrong, while fastidious would always explain you what is right and how it is to be done, this can be further verified by looking at drugs like baryta ,lyco, china, cham, lachesis, calc, ip, caps, cocculus, hyos all these drugs are censorious but not fastidious Sub-headed “critical”. Fault-finding – Censorious also encompasses the modern expression “judgmental”. “Apt to blame or condemn; severe in making remarks on others, or on their writings or manners”. He who is “Censorious” is more concerned with the actions of others as such than is he who is “Fastidious”. 
 
*Conscientious* (about trifles): Equated by Kent with Scrupulous. The patient takes an undue or disproportionate amount of trouble over tasks which cannot be understood as being of more than minor significance. This may be seen in the amount of trouble taken either over the task itself or over making sure that it is completed. (cf. “Trifles seem important”). The word conscientious also describes a person who is strongly guided by a sense of right or wrong, which does things in a way he deems correct. He is honest to himself and to others and would not normally neglect his duties. He is loyal to his job; this person also lies on the verge of being a perfectionist. This is quite close to being “fastidious” and “censorious” but may not be necessarily so. This person is thorough & assiduous and is gifted with extremely caring nature and does great efforts. 
 
Let’s take an example: 
 
Suppose you are late for a meeting, then fastidious person will not tolerate or if you create disorder in room fastidious person will point you out and they’ll point out that don’t do this or that. So seeing superficially he may appear censorious, finding fault but when we go deep, we’ll realize that his concerns are different. His problem is that he cannot be satisfied in respect to time, order or cleanliness or something which he consider important, as he is having his own standards regarding his issues of life, it may be his behavior which may be adapted as in Carcinosin or it may be his temperament and it may or may not involve other person as fastidious is same with himself. But censoriousness is a attitude of person to be critical, fault finding in other and other person may or may not correct in that respect. The censorious or fault finding person is systematically critical of other people, and does not pardon even the slightest faults. He loves to identify shortcomings in other people and to let them know of them. It is not at all for the improvement of the latter as in critic but it is destructive and he is ready to criticize and never praise. Conscientious is totally different from both of them, it is not an attitude of a person but it from inside i.e. By virtue of person, he only listen to his or her conscious and his decision does not depend upon the condition and who is standing in front of him, it is not related with other person it is one own personal understanding and come with teaching and conditioning of a person. 
 
So, this is just a brief idea as to how we should differentiate the rubrics & apply the exact rubric for repertorization & for finding a true similimum. We must know that there are rubics differing in shades of meaning and it may need all our intelligence, care & contemplation to fully realize these shades of meaning.

HYDERABAD: President of the Andhra Pradesh State AYUSH Doctors’ Welfare Association Panuganti Johnson undertook a 12-hour hunger-strike on Friday demanding immediate filling of the 294 vacant posts. He sat on fast at Indira Park. Dr. Johnson said the Government had sanctioned a budget of Rs. 4.39 lakh per annum for the posts, which were yet to be filled. 
 
He lamented that though AYUSH doctors were ready to serve in the rural areas, the Government did not bother to recruit them.

In classical way of homeopathic case analysis, ‘mentals’ and ‘physical generals’ considered to be most important in the selection of similimum. 
 
Normally, while describing ‘general symptoms’, the patient would use the prefix ‘I’, and others would use the prefix ‘He’. 
 
According to the new method of case analysis proposed by Dialectical Homeopathy, ‘general’ symptoms are classified into four categories: 
 
1. Subjective Mental General 
 
2. Subjective Physical General 
 
3. Objective Mental General 
 
4. Objective Physical General 
 
A ‘general symptom’ may be either ‘physical’ or ‘mental’. Further, ‘mentals’ are classified into ‘subjective and ‘objective’. ‘Physical generals’ are also classified into ‘subjective’ and ‘objective’. 
 
 Subjective Mental Generals are those symptoms ‘general mental symptoms’ that could be ‘experienced’ by the patient only. Many hallucinations and delusions belong this group. ‘Grief’, ‘anxiety’ etc are ‘subjective mental generals’. 
 
‘General physical symptoms’ that could be ‘experienced’ only by the patient, such as ‘General weakness’, ‘chilliness’, ‘hot patient’, ‘hot flashes’, etc are ‘subjective physical generals’. 
 
When a ‘subjective physical general’ is associated with a ‘subjective mental general’, it should be graded top in the list, as a ‘subjective mental general’.

If a ‘mental general’ symptom could be observed by others, it becomes a ‘objective mental general’. ‘Laughing immoderately’, ‘wandering on streets’, ‘absence of personal hygiene’, ‘walking hurried’, ‘eating in a hurry’ ‘abusive’, ‘aversion to answer’, ‘bemoaning’, ‘plays antics’, ‘idiotic’, ‘crawling on the floor’ etc are ‘objective mental generals. 
 
‘Objective physical generals’ are ‘physical generals’ that could be observed by others. ‘Obesity’, ‘leanness’, ‘emaciation’, ‘chorea’, ‘chlorosis’, ‘collapse’, ‘fainting’, ‘convulsions’, ‘color of discharges’, ‘dwarfish’, ‘flabbiness of skin’, ‘stoop-shouldered’, ‘anasarca’ , ‘trembling’, ‘positions in sleep’, and such symptoms belong to this group. 
 
If an objective physical general is associated with a symptom belonging to subjective physical general or subjective mental general, it should be ranked top in the list. 
 
After classifying and grading ‘general’ symptoms systematically, we can find an appropriate ‘constitutional similimum’ using all subjective and objective generals.

According to existing methods of homeopathic case analysis, PARTICULAR symptoms come at the lowest strata of symptom hierarchy, and are considered to be of least importance in selecting similimum. Some classical methods even advice to ignore particular symptoms. They say, a perfect homeopathic similimum could be determined on the basis of ‘physical generals’ and ‘mentals’. According to them, prescriptions made on the basis of ‘particular’ symptoms would be superficial, and such prescriptions would provide only palliations, not real cure. 
 
But every homeopathic clinician would testify hundreds of excellent cures made by them using particular symptoms, especially those called ‘uncommon peculiar’. Such experiences obviously show that ‘classical’ approach to evaluation of ‘particular symptoms need some logical modifications. 
 
According to the new method of classifying symptoms proposed by Dialectical Homeopathy, all ‘particular symptoms’ cannot be considered of similar relevance. There are many particular symptoms that can play a decisive role in selecting a similimum.

Particular symptoms are those normally prefixed by ‘MY’ by the patient while narrating symptoms himself. He would say ‘my skin is itching’, my head is aching’, ‘my feet feel numb’, ‘a wart on my fingers’, ‘red eruptions on my palms’ and the like, referring his complaints to some organs and regions of the body. 
 
PARTICULAR symptoms may be SUBJECTIVE PARTICULARS or OBJECTIVE PARTICULARS 
 
Subjective Particulars: 
 
Patients would prefix ‘MY’ when describing symptoms belonging to ‘Subjective Particulars’. Most of the subjective ‘sensations’ referred to any particular region or organ of the body belong to the class of ‘subjective particulars’. They can be only ‘felt’ or ‘experienced’ by the patient. Subjective particulars should be clearly differentiated from ‘Subjective Generals’. 
 
Symptoms such as ‘my skin is itching’, my head is aching’, ‘my feet feel numb’, ‘anxiety in stomach’ etc are ‘Subjective Particulars’. Most of the ‘particular modalities’ such as ‘burning stomach ameliorated by cold drinks’, ‘headache relieved by vomiting’ etc are also ‘Subjective Particulars’. 
 
Objective Particulars: 
 
People observing the patient would normally prefix ‘HIS’ while describing symptoms belonging to ‘Objective Particulars’. ‘ObjectiveParticulars’ are symptoms referred to any particular region or organ of the body which can be observed by the patient, physician or others using their sense organs, with or without the aid of accessory means. Symptoms such as ‘a wart on my fingers’, ‘red eruptions on my palms’ etc are ‘Objective Particulars’. 
 
Many Objective Particular symptoms would be presented in association with a ‘Subjective Particular’ also. Where as ‘red eruptions on feet’ is a n ‘objective particular’, ‘itching eruptions on feet relieved by warmth’ is the associated ‘Subjective Particular’. 
 
When an ‘Objective Particular’ is presented with its associated ‘Subjective Particular’, it becomes a COMPLETE symptom, which is useful for selecting a similimum. Whenever the patient presents an Objective Particular symptom, the physician should inevitably look out for its associated ‘Subjective Particular’ also. An ‘objective particular’ without associated ‘subjective particular’ is of no use in selecting a similimum. If we get an ‘objective particular’ with associated ‘subjective’ particular, it becomes a ‘complete’ symptom, and should be graded in top rank among ‘particulars’. It is classified as ‘Subjective Particular’, not ‘Objective Particular’. 
 
‘Subjective Particular’s may be classified into two groups: 
 
Subjective Mental Particulars 
Subjective Physical Particulars 
 
Objective Particulars are also classified into two groups: 
 
Objective Mental Particulars 
Objective Physical Particulars 
 
While grading ‘PARTICULAR SYMPTOMS’ for repertorization, they should be arranged in following order:

Based on the scientific understanding and explanation of homeopathic therapeutics I propagate, I AM INTRODUCING A NEW CONCEPT OF CASE TAKING, CASE STUDY AND GRADING OF SYMPTOMS, THAT DIFFERS FROM OUR ACCEPTED METHODS. This new concept is only in its evolutionary stage. I am working up on it to make it more perfect and systematic. 
 
According to my view, diseases are deviations in biochemical pathways caused by material level molecular errors in the organism. These deviations are primary factors of diseases. They are expressed through diverse groups of objective and subjective symptoms. Symptoms are only secondary factors. 
 
Classifying and grading of symptoms is a very important step in homeopathic case study and selection of similimum. I am considering a different way of classifying symptoms instead of existing ones. 
 
So far, we were primarily classifying symptoms as ‘physical’ and ‘mental’, considering 'mentals' as decisive factors in selecting similimum. This method of classification is based on the theory that diseases originate in the level of 'vital force'. 
 
According to my observation, this classification is not appropriate, and does not agree with the explanation of homeopathy based on the concept that diseases originate as material level 'molecular errors' in the organism. 
 
There are many ‘physical’ symptoms that are ‘subjective’. ‘Numbness’ is a ‘physical’ symptom, but it is not ‘objective’- it is subjective. ‘Chilliness’ is physical, but subjective.Same way, all ‘mental’ symptoms are not subjective. ‘Laughing immoderately’ is a mental symptom, but it is objective.

According to the new method I propose, we should classify symptoms primarily into: 
 
A. SUBJECTIVE SYMPTOMS 
 
SUBJECTIVE SYMPTOMS are the effects of 'molecular errors' reflected in the consciousness of the individual, mediated by central nervous system. They reflect even minute micro-level errors in the organism. They cannot be observed with our senses with or with out the help of any devices. They can only be 'felt' or 'experienced' by the individual. SUBJECTIVE symptoms play a decisive role in identifying exact molecular errors and selecting the appropriate similimum. 
 
B. OBJECTIVE SYMPTOMS 
 
OBJECTIVE SYMPTOMS are the gross tissue level reflections of underlying 'molecular errors'. They are observed by the individual himself, the physician or the family members, with sense organs aided or unaided by any devices. Observations through physical examinations and laboratory investigations also belong to this class. 
 
Observing objective symptoms along with associated subjective symptoms help us to identify the exact molecular errors, which is necessary for selecting appropriate similimum. 
 
A. SUBJECTIVE SYMPTOMS can be then classified into ‘subjective mentals’ and ‘subjective physicals’. 
 
‘Subjective mentals’ can be divided into ‘subjective mental generals’ (SMG) and ‘subjective mental particulars’ (SMP) 
 
‘Subjective physicals’ can be classified into ‘subjective physical generals’ (SPG) and ‘subjective physical particulars’ (SPP). 
 
B. OBJECTIVE SYMPTOMS can be classified into ‘objective mentals’ and ‘objective physicals’ 
 
 
‘Objective mentals’ can be divided into ‘objective mental generals’ (OMG) and ‘objective mental particulars’ (OMG). 
 
‘Objective physicals’ can be classified into ‘objective physical generals’ (OPG) and ‘objective physical particulars’ (OPP). 
 
According to their importance in selecting similimum, we can list symptoms in the following order: 
 
 
1. SMG= SUBJECTIVE MENTAL GENERAL 
2. SMP= SUBJECTIVE MENTAL PARTICULAR 
 
3. SPG= SUBJECTIVE PHYSICAL GENERAL 
4. SPP= SUBJECTIVE PHYSICAL PARTICULAR 
 
5. OMG= OBJECTIVE MENTAL GENERAL 
6. OMP= OBJECTIVE MENTAL PARTICULAR

During CASE TAKING, when we observe an OBJECTIVE symptom, we should look for associated SUBJECTIVE symptom also. Then only it becomes a COMPLETE symptom. For example, if ‘eruptions on skin’ is a OPP, ‘itching amel by warmth’ may be associated SPP. Then it becomes a COMPLETE symptom. All symptoms we call modalities, sensations and concomitants would belong to any of these categories. Obviously, a modality may be subjective or objective, physical or mental, general or particular. Its importance also vary accordingly. 
 
‘Always lying down’ may be a OPG. Its associated SMG may be ‘desire to lie’, or SPG may be ‘weakness’. 
 
‘Anxiety while eating’ is a SMP. ‘Eating in a hurry’ may be its associated OMP. 
 
‘Numbness of lower limbs’ is a SPP. ‘Numbness amel by motion’ may be associated SPP. ‘restless limbs’ may be associated OPP. 
 
When OPP is restless limbs, associated SPP may be ‘pain limbs amel by motion’, or it may be associated with an SPG ‘restlessness’, or SMG ‘anxiety’. 
 
SMG, SPG, OMG and OPG jointly decide the CONSTITUTION of an individual. 
 
If you follow this new method of CASE TAKING and GRADING OOF SYMPTOMS, selection of similimum becomes more easy and accurate. I have found excellent outcomes by following this method

Homeopaths consider ‘susceptibility’ of the individual as the fundamental ‘cause’ of disease. According to them, ‘susceptibility’ is a property of ‘vital force’, which is a ‘dynamic’,‘non-material’, ‘non-corporeal’, ‘conceptual’ and spiritual entity that enlivens and governs the organism from the ‘interior’. As such, ‘susceptibility’ to diseases is also ‘dynamic’. 
 
As per this concept, ‘classical’ homeopaths would persistently argue that even so-called ‘infectious diseases’ are not caused by bacteria or viruses, but the ‘internal susceptibility’, dynamic in nature. They say: "Small pox virus is not the cause of smallpox, vibrio cholerae is not the cause of cholera". According to this theory, homeopathy is not involved with ‘treating infections’, but ‘correcting’ the susceptibility. 
 
‘Constitutional susceptibility’ and ‘miasmatic susceptibility’ are terms frequently encountered in homeopathic discussions. 
 
 
Similar to ‘vital force’ theory regarding diseases, and ‘dynamization’ theory of potentized drugs, this ‘dynamic’ theory of ‘susceptibility’ is another stumbling block that prevents homeopathy from becoming a scientific medical system. With such a totally unscientific theory homeopathy cannot effectively communicate with scientific community. 
 
‘Susceptibility’ in medical terms means the ‘state or character of being susceptible to disease’. 
 
In epidemiology “a susceptible individual is a member of a population who is at risk of becoming infected by a disease, or can not take a certain medicine, antibiotic, etc if he or she is exposed to the infectious agent.” 
 
From the viewpoint of immunology, ‘susceptible’ individuals have been exposed to neither the wild strain of the disease nor a vaccination against it, and thus have not developed immunity. Those individuals who have antibodies against an antigen associated with a particular infectious disease will not be susceptible, even if they did not produce the antibody themselves (for example, infants younger than six months who still have maternal antibodies passed through the placenta and from the colostrum, and adults who have had a recent injection of antibodies). However, these individuals soon return to the susceptible state as the antibodies are broken down. 
 
Some individuals may have a genetically determined natural resistance to a particular infectious disease. However, except in some special cases, these individuals make up such a small proportion of the total population 
 
In Virology, ‘susceptibility’ is an important factor that decides the possibility of viral infections. Viruses are only able to cause disease or pathologies if they meet several criteria: 1. The virus is able to enter the cell (called a susceptible state). 2. There is a sufficient number of viruses within the cell. 3. The virus is able to replicate within the cell (called a permissive state). Hence ‘susceptibility’ only refers to the fact that the virus is able to get into the cell, via having the proper receptor(s), and as a result, despite the fact that a host may be susceptible, the virus may still not be able to cause any pathologies within the host. Reasons for this are varied and may include suppression by the host immune system, or abortive measures taken by intrinsic cell defenses.” All these factors belong purely to biochemistry- nothing ‘dynamic’ 
 
There is no doubt, ‘susceptibility’ plays a major role in disease processes. When we sow same seeds on different fields, their rate of germination and growth will be different. The environmental conditions of soil, atmosphere and climate play a decisive role in this process. Availability of nutrients, water, sunlight, and various other micro-level factors influence the germination of seeds and growth of seedlings. If we sow same seeds on a barren rock, it will not germinate. This shows the role of environmental factors. 
 
In a similar way, the internal biochemical environment of the organism, which is also more or less influenced by external environment, plays a role in deciding the ‘susceptibility’ of the individual to diseases including infections. ‘Causative’ agents of diseases are expressed in a biochemical background of ‘susceptibility’.

HOMEOPATHIC 'CONSTITUTIONS' EXPLAINED IN TERMS OF 'GENOTYPE-PHENOTYPE' INTERACTIONS STUDIED BY MODERN GENETICS

‘Constitutions’, ‘constitutional symptoms’ and ‘consitutional drugs’ are concepts which play a very important role in homeopathic theory and practice. Concepts such as ‘genetic constitution’ and ‘miasmatic constitution’ are frequently heard in homeopathic discussions. There have been a lot of attempts to explain constitution in terms of 'miasms', genetics, embryology and many other concepts. 
 
I am trying to evolve a scientifically viable understanding of our concept of 'constitution'. I think it would be more logical and scientific if we understand 'constitution' in terms of ‘phenotypes’ of individuals. To understand and explain ‘constitutions’ in scientific terms, we have to understand the concepts of ‘genotypes’ and ‘phenotypes’ in modern genetics. 
 
According to modern genetics, the ‘genotype’ is the ‘genetic substance or ‘DNA’ inherited by the organism from its previous generation. It is called the ‘genetic blue print’. 
 
The ‘genotype’ contained the organism gives rise to individual ‘phenotypes through ‘gene expressions’. The ‘genetic code’ stored in DNA is interpreted by ‘gene expression’, and the properties of these expressions five rise to the ‘phenotype’ of the organism. 
 
 
A ‘phenotype’ is the observable characteristics or traits of an organism, such as morphology, development, biological and physiological properties, behavior, and products of behavior. ‘Phenotype’ is the result of ‘gene expressions’, which is decided by the interaction between genetic blue print and environmental factors. 
 
‘Genotype’ of an organism is the inherited instructions it carries within its genetic code. Organisms with same ‘genotype’ do not appear or act the same way, because its ‘phenotype’ is decided by the interaction with environmental and developmental conditions. Similarly, not all organisms that look alike necessarily have the same genotype. 
 
This understanding of ‘genotype-phenotype distinction’, proposed by Wilhelm Johannsen in 1911 to make clear the difference between an organism's heredity and what that heredity produces, is very important in providing a scientific explanation for the homeopathic concept of ‘constitutions’. 
 
Despite its seemingly straightforward definition, the concept of the phenotype has some hidden subtleties. Some would argue that anything dependent on the genotype is a phenotype, including molecules such as RNA and proteins. Most of the molecules and structures coded by the genetic material are not visible in the appearance of an organism, yet they are observable and are thus part of the phenotype. Human blood groups are an example. Others would say that this goes beyond the original intentions of the concept with its focus on the (living) organism in itself, meaning that the lowest level of biological organization compatible with the phenotype concept is at the cellular level. Either way, the term phenotype includes traits or characteristics that can be made visible by some technical procedure. Another extension adds behavior to the phenotype since behaviors are also observable characteristics. Indeed there is research into the clinical relevance of behavioral phenotypes as they pertain to a range of syndromes. 
 
Phenotypic variation (due to underlying heritable genetic variation) is a fundamental prerequisite for evolution by natural selection. It is the living organism as a whole that contributes (or not) to the next generation, so natural selection affects the genetic structure of a population indirectly via the contribution of phenotypes. Without phenotypic variation, there would be no evolution by natural selection. 
 
The relationship between ‘genotype’ and ‘phenotype’ has often been conceptualized by the following relationship: “genotype (G) + environment (E) + genotype & environment interactions (GE) → phenotype (P)” 
 
‘Genotypes’ often have much flexibility in the modification and expression of phenotypes; in many organisms these phenotypes are very different under varying environmental conditions. The concept of phenotype can be extended to variations below the level of the gene that affect an organism's fitness. For example, silent mutations that do not change the corresponding amino acid sequence of a gene may change the frequency of guanine-cytosine base pairs (GC content). These base pairs have a higher thermal stability than adenine-thymine, a property that might convey, among organisms living in high-temperature environments, a selective advantage on variants enriched in GC content. 
 
A phenotype is the ensemble of observable characteristics displayed by an organism. The idea of the phenotype expresses all the effects a gene has on the outside world that may influence its chances of being replicated. These can be effects on the organism in which the gene resides, the environment, or other organisms. 
 
Gene expression is the process by which information from a gene is used in the synthesis of a functional gene product. These products are often proteins, but in non-protein coding genes such as ribosomal RNA (rRNA), transfer RNA (tRNA) or Small nuclear RNA (snRNA) genes, the product is a functional RNA. The process of gene expression is used by all known life to generate the macromolecular machinery for life. 
 
Several steps in the gene expression process may be modulated, including the transcription, RNA splicing, translation, and post-translational modification of a protein. Gene regulation gives the cell control over structure and function, and is the basis for cellular differentiation, morphogenesis and the versatility and adaptability of any organism. Gene regulation may also serve as a substrate for evolutionary change, since control of the timing, location, and amount of gene expression can have a profound effect on the functions (actions) of the gene in a cell or in a multicellular organism.
